The following are the specific eligibility requirements. Services under the Louisiana Children's Choice are available only to individuals who meet all of the following criteria. The child must continue to meet all waiver eligibility criteria to remain eligible.

AGE

• The age range for Louisiana Children's Choice is birth through 18 years of age. Children will be transferred to another appropriate NOW waiver when they reach age 19 if they continue to meet all waiver eligibility criteria.

SLOT AVAILABILITY

• Child's name on the statewide NOW waiver waiting list.

Referrals for support coordination services are made only by BCSS.
